Senior Citizens in Brampton can access free homecare through new Osler program

Senior Citizens in Brampton can get months of free at-home care after they are discharged from the hospital via a new home care program as William Osler Health System has collaborated with Bayshore HealthCare Integrated Care Solutions for the Osler@Home program to help senior citizens leaving hospital transition back home.

Osler started accepting patient referrals for the new Osler@Home program recently and approved patients will get 16 weeks of provided care.

The program seeks to assist with patient recovery while addressing hospital capacity challenges, particularly among senior citizens that no longer need speciality hospital services but still need help to recover successfully at home.

In a release, Osler said staff of Brampton Civic Hospital will spot eligible patients and meet with them and their families to establish a customized care plan for every patient to support “a seamless hospital discharge and a continuous care experience at home.”

Clint Atendido, VP of clinical services with the health system, said: “We are proud to partner with Bayshore HealthCare Integrated Solutions to bring this innovative program to our patients, and support recovery at home with the expert care of Bayshore staff, supported by Osler.”

Osler, oversees operations at Peel Memorial Centre and Brampton Civic Hospital. It has launched the program at Etobicoke General Hospital.

Danielle Holler, Regional Director of Bayshore, said: “We are thrilled to partner with William Osler Health System to provide patients and families in the Brampton and North Etobicoke community with timely access to quality healthcare services in the comfort of their home.”
